An employee was monitoring packages being filled on a vertical form fill machine when the machine jammed. While she was trying remove the jam, the blades closed on her left hand. She suffered third degree burns and contusions on her left hand and fingers.

An employee was unloading merchandise and cutting open a pallet of boxes stacked 7 boxes high. Some of the boxes fell on top of the employee and knocked him 20 feet to the concrete floor. The employee had surgery to repair a broken ankle and lower knee fracture.
